A chronic condition management plan helps you and your GP plan and review care for a long-term health condition. Here are five essential things to know about the new plans.

If you live with a long-term health condition such as diabetes, heart disease, asthma or arthritis, a care plan with your GP can help you stay on top of your health. From 1 July 2025, Medicare replaced GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) with a single GP Chronic Condition Management Plan (GPCCMP).
What is a chronic condition management plan?
It is a written plan prepared by your GP for a chronic (long-term) or terminal medical condition. It sets out your health goals, the care and services you need, and who will provide them. Your GP reviews the plan with you regularly so it stays up to date.
Who is eligible?
Generally, the plan is for people with a medical condition that has been present, or is likely to be present, for six months or longer, or that is terminal. Your GP will assess whether a plan is right for you.
What if I already have a GPMP or TCA?
If you had a GPMP or TCA prepared before 1 July 2025, you don’t need to do anything straight away. Your GP will talk with you about moving to the new plan at your next review. Ask our team if you’re unsure what applies to you.
Access to allied health
If you are eligible, your plan may allow you to claim Medicare rebates for a number of allied health services each calendar year, such as physiotherapy, dietetics, podiatry or psychology. At our clinic, allied health professionals work alongside your GP, which makes coordinated care easier.
MyMedicare
If you are registered with MyMedicare at a practice, your plan is generally prepared and reviewed by that practice. Find out more about MyMedicare enrolment at Teesdale Family Medical Centre.
What to bring to your appointment
- Your Medicare card
- A list of your current medicines
- Recent test results or letters from specialists
- A few notes about your goals and what you find hardest to manage
Care planning appointments are usually longer, so please let reception know when booking.
Getting the most from your chronic condition management plan
Your plan works best when it reflects what matters to you. Before your appointment, think about the goals you would like to work towards – for example, keeping your blood sugar in range, being able to walk further, sleeping better or reducing pain. Bring any home readings such as blood pressure or glucose results. Between reviews, let your GP know if anything changes, such as new symptoms, a hospital stay or side effects from medicines.
Regular reviews help pick up problems early and keep your care up to date. Many people find it helpful to book their next review before leaving the clinic. You can read more about chronic conditions on healthdirect.
Chronic condition management plan – common questions
Do I need a new plan every year?
Your plan is reviewed regularly rather than started from scratch. Your GP will let you know how often reviews are needed for your condition.
Can a practice nurse help with my plan?
Practice nurses often help with parts of care planning, such as measurements and education, working alongside your GP, who remains responsible for the plan.
